Patient Consents
Patient Intake
We are committed to providing high-quality psychiatric care with transparent pricing. We operate as an out-of-network provider, meaning payment is due at the time of service. However, we can provide you with a superbill that you may submit to your insurance for potential reimbursement.
Appointment Fees
Initial Psychiatric Assessment & Interview (60-75 minutes) – $425
Follow-Up & Medication Management (30 minutes) – $225
We accept payment via credit/debit cards, HSA/FSA accounts, and other secure payment methods.
New Client Services
Enrollment
For any new clients, we offer an initial no-cost, brief phone screen with our Practice Manager that can be conducted prior to scheduling the initial diagnostic evaluation session to determine the appropriateness of the referral question, likelihood of fit for our services, and to answer other questions. For many clients, this phone screen will not be necessary but remains an available service for anyone interested.
Prescription Services
Our provider is fully licensed to prescribe psychiatric medications, including controlled (scheduled) substances when clinically appropriate. All prescriptions are sent electronically to the pharmacy of your choice for your convenience.
Insurance
Emergence Treatment and Research does not accept insurance for outpatient services. This means you will pay the agreed-upon fee at the time of service with us and you will be provided with the documentation required by most insurance companies for reimbursement.
Most of our clients have insurance and many want to use it to cover some or most of the costs of treatment. Our clients often take advantage of filing with insurance as many managed care organizations (MCOs) allow for reimbursement of a percentage of fees for an “out-of-network” OON provider. Whether you choose to file for reimbursement with your insurance company is an individual decision. We are happy to talk with you further about these issues upon request.
*Services reimbursed by insurance can vary. It is your responsibility to check with your individual policy as we are considered “Out of Network” providers.
Policies for new services
For all new clients and returning clients who have not been seen by a therapist at Emergence Treatment and Research for more than two (2) years at our practice, we conduct an initial diagnostic evaluation, charged at the rate of $425. This evaluation includes a clinical interview lasting 60-75 minutes
Policy for Ongoing Psychotherapy and Medication Management Services
Upon completion of your assessment, your provider will then determine if medication interventions are appropriate and how frequently they want to see you. Prescriptions will be sent electronically to your pharmacy of choice. The provider will send prescriptions to cover you until you are expected to return for a follow-up appointment. Providers will not send in refill requests for patients who fail to schedule their follow-up appointment.
Follow-up Psychotherapy and Medication Management Appointments are scheduled for 30 to 45 minutes are the cost is $225.
Other Fees:
Emergence Treatment and Research offer a prorated fee schedule, in 15-minute increments for additional time outside of intake or follow-up appointments maximum appointment time. This also includes any of the following:
Requested report writing, letters of opinion, or other documentation services not already included in assessment services
Phone Consultations with school or other professionals, longer than 10 minutes
Review of lengthy emails or correspondence provided by the client
These are charged at $50 per increment of time and patients are fully informed of this cost and expected time prior to the service so there is full transparency with financial costs.
Methods of Payment for Services
We accept cash, check, or credit card (Visa, MC, Discover, and Amex) as well as Debit cards and prepaid cards that are funded through Health Savings and Flex Spending accounts.
Our credit card processing is handled online through the secure Car Pointe Portal. Our office staff will reach out on the day of your appointment to process fees.
Any returned checks or late payments may be subject to a $35 fee. Noncompliance with payment terms may incur collections charges.
Any refunded credit card charges may be subject to a fee of 5% of the total transaction.
Cancellation Policy
To remain available for all our clients, we request that cancellations be made at least 24 hours in advance. Appointments that are not canceled in advance of 24 hours, and that are not due to a death in the family, illness, or emergency situations will incur the following charges:
1st missed appointment will be charged at 50% of the rate of the missed appointment
2nd missed appointment and thereafter will be charged at the full rate of the missed appointment
Cancellation charges will be applied and processed at your next appointment unless
payment is made online in advance.
